and efficacy of treatment in patients with LC in the course of BCLL are controversial. Many authors describe poor prognosis, when LC shows blastic transformation, known as Richter’s syndrome, and when it appears after the diagnosis of B-CLL. The potential benefit of treating only skin lesions has not been documented; however, these patients rarely stay without treatment. Numerous therapeutic modalities have been described. Disseminated skin infiltrates are treated with alkylating agents, cladribine and UVB phototherapy. In localized lesions, radiotherapy and intralesional injection of interferon have been tried out. Methotrexate is an effective treatment for different lymphoproliferative disorders both as systemic or intralesional treatment. Intralesional methotrexate has also been used in the treatment of keratoacanthoma. This therapy appears to be a simple, relatively safe and effective alternative to other therapeutic modalities. However, complete blood cell controls are necessary as pancytopenia have been reported as side effect. Larger studies and more prolonged follow-up are needed to establish its interest.
